Model Session Booking 7 Model Session Booking Form Important: This form is to only be filled out on being invited by Jade to participate in a model session photoshoot. The Model understands and agrees to the following: The Model must not be a photographer or be involved in any way in photography business. The Model agrees to make themselves and their newborn available on the day of the model session at their allocated time which is advised by The Photographer. The Model agrees that if their newborn is sick or has any conditions which could cause issues or discomfort for the baby attending a photoshoot that they will seek advice from their doctor prior to the session. In return for the use of the Model’s images for the sale of online training programs and any other commercial use by the Photographer, the Model will be provided with 10 digital hires images which will be supplied normally around 3 weeks after the session. If there is a delay in supply of the images the Photographer will advise the Model. The Photographer retains the copyright of all images created during the session (including digital files and previews) and reserves the right to use and reproduce the images for advertising, display on their websites, publication and other commercial purposes. No photos or videos may be taken by the Model during the session. The model reserves their place as the Model by paying a $100 deposit which is then refunded on the day of the model session as long as the Model attends the model session. If the Model does not attend the session for any reason then the $100 deposit will be forfeited and not returned to the Model. Jade does her best to be available to complete a model session for every model session booking. If however Jade is unable to do your session due to urgent commitments, unforeseen circumstances, sickness, factors outside of Jade’s control or for any other reason then you agree that your model session may be cancelled and that the maximum and only liability the Photographer will have to you is to refund your model session deposit of $100.
